This is a recent specialty in Togo, with a multi-skilled anesthetic team. To describe the anesthetic management and morbidity of thoracic surgery. A descriptive, prospective and observational study was conducted on a cohort of patients who underwent a thoracic surgery between June 1 and August 31, 2022, at the national referral hospital in Lomé. The study examined pre-anesthetic assessment, surgical, anesthetic, and postoperative data. Twenty-five patients with a mean age of 40 ±13 years were included. The surgery was elective in 69% of cases. The anesthetic assessment showed anemia (64%) and a reduced forced expiratory volume in one second (60%) All patients were operated on under general anesthesia with controlled ventilation, including 64% of one-lung ventilation. Surgical procedures included pleural decortication (28%), pericardial drainage (16%), pneumonectomy (16%), and pulmonary lobectomy (12%). Twenty-three patients (92%) experienced intraoperative complications, including arterial hypotension (80%), shock (56%), and hypoxia (24%). Multimodal analgesia including paracetamol (100%), nefopam (92%), morphine (76%), paravertebral analgesia (20%), and thoracic epidural analgesia (8%), was used postoperatively. Seventeen patients (68%) experienced postoperative complications, including anemia (20%), pneumonia (12%), and parietal infection (12%). Three patients (12%) died. General anesthesia with, one-lung ventilation in most cases, was the anesthetic technique in thoracic surgery. Complications, mainly cardiovascular, occurred intraoperatively, with high postoperative mortality. Anesthetic management and morbidity in thoracic surgery: Results of the first series in Togo · PMID 38372517Hypoxemia is considered to be the most important challenge during one-lung ventilation. Recent studies, however, have shown that one-lung ventilation can involve some lung damage and can therefore be per se a cause of hypoxemia. It has been shown that some parameters of one-lung ventilation are associated with an increased probability of lung injury. High tidal volumes can trigger an increase in some inflammatory mediators in both experimental and clinical settings. High inspiratory pressures and/or a collapse of alveoli in every respiratory cycle would lead not only to an impairment of oxygenation, but also to a further incidence of 'postpneumonectomy pulmonary edema', the earlier definition of lung injury associated with one-lung ventilation. Hypoxemia should always be considered as the most important challenge during one-lung ventilation. One should also keep in mind, however, that some ventilatory strategies can even be harmful. Lung isolation involves the anatomical or physiological separation of 1 lung from the other using advanced airway devices such as bronchial blockers or double-lumen endotracheal tubes. Anatomical lung isolation is crucial for separating the diseased lung and preventing contamination of the healthy lung. For example, in the case of whole lung lavage, anatomical lung isolation is used to avoid spillage of blood or pus from the diseased to the healthy lung. Physiological lung separation allows each lung to be ventilated independently and may be indicated when unilateral lung pathology requires different ventilatory parameters for each lung. One-lung ventilation involves ventilating 1 lung while allowing the other to collapse, thereby facilitating surgical exposure or managing disease states. The deflation of 1 lung can create an immobile surgical field and provide space within the thorax, allowing access to anatomical structures.  A clinician with advanced airway training (eg, an anesthesiologist or intensivist) should place the double-lumen endotracheal or bronchial blocker. Extensive knowledge of pulmonary physiology is essential for managing lung isolation, particularly during periods of one-lung ventilation, which can be complicated by hypoxia. When used to facilitate surgery, lung isolation requires clear communication between the surgical and anesthesiology teams to prevent and manage complications. In the intensive care unit (ICU) setting, nurses and respiratory therapists must be aware of the physiological consequences of differential or one-lung ventilation and detect changes in the patient's condition that may indicate a problem with lung isolation.

Ventilatory management of one-lung ventilation · PMID 21540809Acute respiratory distress syndrome (ARDS) is noncardiogenic pulmonary edema that manifests as rapidly progressive dyspnea, tachypnea, and hypoxemia. Diagnostic criteria include onset within one week of a known insult or new or worsening respiratory symptoms, profound hypoxemia, bilateral pulmonary opacities on radiography, and inability to explain respiratory failure by cardiac failure or fluid overload. ARDS is thought to occur when a pulmonary or extrapulmonary insult causes the release of inflammatory mediators, promoting inflammatory cell accumulation in the alveoli and microcirculation of the lung. Inflammatory cells damage the vascular endothelium and alveolar epithelium, leading to pulmonary edema, hyaline membrane formation, decreased lung compliance, and decreased gas exchange. Most cases are associated with pneumonia or sepsis. ARDS is responsible for one in 10 admissions to intensive care units and one in four mechanical ventilations. In-hospital mortality for patients with severe ARDS ranges from 46% to 60%. ARDS often must be differentiated from pneumonia and congestive heart failure, which typically has signs of fluid overload. Treatment of ARDS is supportive and includes mechanical ventilation, prophylaxis for stress ulcers and venous thromboembolism, nutritional support, and treatment of the underlying injury. Low tidal volume and high positive end-expiratory pressure improve outcomes. Prone positioning is recommended for some moderate and all severe cases. As patients with ARDS improve and the underlying illness resolves, a spontaneous breathing trial is indicated to assess eligibility for ventilator weaning. Patients who survive ARDS are at risk of diminished functional capacity, mental illness, and decreased quality of life; ongoing care by a primary care physician is beneficial for these patients.
Acute Respiratory Distress Syndrome: Diagnosis and Management · PMID 32538594Despite the physiological advantages of positive end-expiratory pressure (PEEP), its optimal utilization during one-lung ventilation (OLV) remains uncertain. We aimed to investigate whether individualized PEEP titration by lung compliance is associated with a reduced risk of postoperative pulmonary complications during OLV. We searched PubMed, Embase, and the Cochrane Central Register of Controlled Trials until April 1, 2024, to identify published randomized controlled trials that compared individualized PEEP titration by lung compliance with fixed PEEP during OLV. The primary outcome was a composite of postoperative pulmonary complications. Secondary outcomes included clinical outcomes (pneumonia, atelectasis, ARDS, cardiovascular complications, mortality), respiratory mechanics, gas exchanges, and hemodynamic variables. Subgroup analyses were conducted for the primary outcome according to the PEEP titration method (dynamic compliance vs. driving pressure/static compliance, stepwise decremental vs. incremental strategy). Ten trials involving 3426 patients were included. Compared with fixed PEEP, individualized PEEP titration by lung compliance was associated with reduced risk of a composite of postoperative pulmonary complications (eight trials, 3351 patients, risk ratio [RR] 0.55, 95% CI 0.38-0.78). Subgroup analyses suggested the association was evident in the subgroup with titration by dynamic compliance rather than driving pressure/static compliance and in the subgroup with PEEP titration by stepwise decremental but not stepwise incremental strategy. Individualized PEEP titration by lung compliance was also associated with a reduced risk of pneumonia (RR 0.71, 95% CI 0.52-0.96) and atelectasis (RR 0.63, 95% CI 0.45-0.88), higher dynamic compliance, PaO2, PaO2/FiO2, and lower driving pressure. The individualized and fixed PEEP groups did not differ in ARDS, cardiovascular complications, mortality, peak pressure, plateau pressure, PaCO2, heart rate, and mean arterial pressure. Compared with fixed PEEP, individualized PEEP titration by lung compliance is associated with a reduced risk of postoperative pulmonary complications during OLV, especially in PEEP titration by dynamic compliance or stepwise decremental strategy. Individualized PEEP titration by lung compliance during one-lung ventilation: a meta-analysis · PMID 39825438The effect of higher positive end-expiratory pressure (PEEP) and recruitment manoeuvres aimed at lung expansion as compared with lower PEEP without recruitment manoeuvres aimed at permissive atelectasis on postoperative pulmonary complications (PPCs) in patients undergoing one-lung ventilation (OLV) during thoracic surgery is unclear. We aimed to determine the contribution of an intraoperative lung expansion strategy to preventing PPCs. In this multicentre, randomised, controlled, international phase 3 trial (PROTHOR) conducted at 74 sites in 28 countries, we enrolled adult patients (aged ≥18 years) with a BMI of less than 35 kg/m2 who were scheduled for open thoracic or video-assisted thoracoscopic surgery under general anaesthesia requiring one-lung ventilation with a double-lumen tube, with a planned operative time of more than 60 min, and an expected duration of one-lung ventilation longer than that of two-lung ventilation. Patients were randomly assigned (1:1), using permuted blocks of random size (4, 6, and 8) and stratified by study site, to receive one-lung ventilation with either a higher PEEP of 10 cm H2O and periodic lung recruitment manoeuvres (high PEEP group) or a lower PEEP of 5 cm H2O without routine recruitment manoeuvres (low PEEP group). All patients received protective tidal volumes of 5 mL/kg predicted body weight during one-lung ventilation and 7 mL/kg predicted body weight during two-lung ventilation. Postoperative assessors were masked to treatment allocation. The primary outcome was a composite of PPCs during the first 5 postoperative days, including aspiration pneumonia, moderate or severe respiratory failure, acute respiratory distress syndrome (ARDS), pulmonary infection, atelectasis, cardiopulmonary oedema, pleural effusion, non-operative pneumothorax, pulmonary infiltrates, prolonged air leak, purulent pleuritis, pulmonary embolism, and pulmonary haemorrhage. A modified intention-to-treat analysis was performed, with patients analysed according to their assigned treatment group, except in cases of withdrawal of informed consent, cancellation of surgery, and or loss to follow-up. This trial is registered with ClinicalTrials.gov (NCT02963025) and is completed. Between Jan 3, 2017, and Feb 12, 2024, 2200 patients were randomly allocated: 1099 to the high PEEP group and 1101 to the low PEEP group. 43 patients in the high PEEP group and 33 in the low PEEP group were excluded from the modified intention-to-treat analysis after randomisation. The primary outcome occurred in 555 (53·6%) of 1036 patients in the high PEEP group and 592 (56·4%) of 1049 patients in the low PEEP group (absolute risk difference -2·68 percentage points [95% CI -6·36 to 1·01]; p=0·155). Intraoperative complications occurred in 484 (49·8%) of 972 patients in the high PEEP group and in 305 (31·3%) of 974 patients in the low PEEP group (absolute risk difference 18·09 percentage points [95% CI 14·41-21·77]), among which hypotension (360 [37·3%] of 966 patients in the high PEEP group vs 140 [14·3%] of 978 in the low PEEP group) and new arrhythmias (89 [9·9%] of 899 vs 37 [3·9%] of 956) were more frequent in the high PEEP group, while hypoxaemia rescue manoeuvres were more frequent in the low PEEP group (29 [3·3%] of 888 vs 86 [8·8%] of 982). The proportions of patients with extrapulmonary postoperative complications (110 [10·6%] of 1036 vs 107 [10·2%] of 1049 patients), and the numbers of adverse events (209 vs 204 events), did not differ between groups. In patients with a BMI of less than 35 kg/m2 undergoing thoracic surgery, one-lung ventilation using higher PEEP with recruitment manoeuvres, compared with lower PEEP without recruitment manoeuvres, did not reduce PPCs. The choice for intraoperative lung expansion or permissive atelectasis should take the individual gas-exchange and haemodynamic conditions into account, which might vary during the intraoperative period. Effects of intraoperative higher versus lower positive end-expiratory pressure during one-lung ventilation for thoracic surgery on postoperative pulmonary complications (PROTHOR): a multicentre, international, randomised, controlled, phase 3 trial · PMID 41240959The potential for the anti-inflammatory effects of lidocaine to reduce complications after lung resection has not been evaluated. We assessed whether intraoperative i.v. or paravertebral lidocaine, compared with remifentanil, reduced complications after lung resection. We also quantified pulmonary and systemic inflammatory responses. This single-centre study randomised participants undergoing lung resection (video-assisted thoracoscopy) to receive continuous infusion of either (1) i.v. lidocaine and paravertebral saline, (2) paravertebral lidocaine and i.v. saline, or (3) remifentanil and paravertebral saline. The primary outcome was the proportion of participants with all-cause complications (Clavien-Dindo classification). Cytokines were measured in bronchoalveolar lavage fluid before and after one-lung ventilation and in plasma during the first 24 h. In 154 patients (mean age 65 yr; 42% female), fewer major (Clavien-Dindo Grade ≥III) complications were recorded in participants randomised to intraoperative paravertebral (two of 49; 4.1%) or i.v. (two of 54; 3.7%) lidocaine, compared with six of 51 (11.8%) participants who received remifentanil (P=0.037). Severe complications were less likely with lidocaine from either i.v. or paravertebral sources, compared with remifentanil (odds ratio: 0.44 [95% confidence interval: 0.22-0.88]). Pulmonary complications occurred in 23/103 (22.3%) participants allocated to intraoperative lidocaine from either i.v. or paravertebral routes, compared with 23/51 (45.1%) participants who received remifentanil (odds ratio: 0.35 [95% confidence interval: 0.17-0.72]; P=0.004). Cytokine levels after one-lung ventilation were lower with lidocaine treatment. Intraoperative lidocaine infusion administered either intravenously or through paravertebral catheter reduces the incidence and severity of complications after lung resection surgery compared with remifentanil. Effect of intraoperative paravertebral or intravenous lidocaine infusion on postoperative complications and inflammation after lung resection surgery: a randomised controlled trial · PMID 40897588Comparison of flow-controlled ventilation (FCV) to standard of pressure-controlled ventilation (PCV) in thoracic surgery procedures requiring one-lung ventilation. Prospective, non-blinded, randomized, controlled trial. Operating theater at a university hospital, Austria. Patients scheduled for elective, thoracic surgery. Participants received ventilation randomly either with FCV or PCV per-protocol for the duration of anesthesia. The primary endpoint was oxygenation assessed by paO2 / FiO2 ratio 30 min after the start of OLV. Secondary endpoints included the required minute volume for CO2 removal, applied mechanical power and incidence of postoperative pulmonary complications. A total of 46 patients were enrolled and 43 included in the primary analysis. The primary endpoint paO2 / FiO2 ratio was significantly higher in the FCV group (n = 21) compared to the control group (PCV n = 22) (187 vs 136 mmHg, MD 39 (95 % CI 1 to 75); p = 0.047). The required respiratory minute volume to obtain comparable mild hypercapnia during OLV was significantly lower in FCV (3.0 vs 4.5 l/min, MD -1.3 (95 % CI -1.9 to -0.8); p < 0.001). The applied mechanical power was also significantly lower (3.5 versus 7.6 J/min, MD -3.8 (95 % CI -5.3 to -2.7); p < 0.001). In this single-center randomized controlled trial, flow-controlled ventilation improved gas exchange parameters in terms of oxygenation and carbon dioxide removal during one-lung ventilation in patients undergoing thoracic surgery and reduced the mechanical impact of artificial ventilation.
Flow-controlled versus pressure-controlled ventilation in thoracic surgery with one-lung ventilation - A randomized controlled trial · PMID 40020438One-lung ventilation (OLV) during thoracoscopic surgery can impair oxygenation due to increased intrapulmonary shunts. Remimazolam has gained attention for its hemodynamic stability and rapid recovery profile; however, its effects on arterial oxygenation during OLV remain unclear. This study aimed to compare the effects of remimazolam and sevoflurane on arterial oxygenation during OLV. In this prospective, randomized controlled trial, 58 adult patients undergoing thoracoscopic surgery were assigned to receive either sevoflurane or remimazolam for anesthesia maintenance. Arterial blood gas analysis and hemodynamic parameters were measured at four time points: 10 min after lateral positioning during two-lung ventilation (TLV10) and 15, 30, and 60 min after initiating OLV (OLV15, OLV30, and OLV60). The primary outcome was arterial partial pressure of oxygen (PaO2) at OLV30. Secondary outcomes included time-dependent changes in PaO2, hemodynamic variables, and serum lactate levels. No significant difference in PaO2 at OLV30 was observed between groups (108.9 ± 37.9 vs. 107.0 ± 37.8 mmHg, 95% CI [-21.8 to 18.0], P = 0.815). In within-group analysis, PaO2 at OLV60 increased significantly from TLV10 in the remimazolam group (95% CI [0.3-36.8], P = 0.044), while no such improvement was observed in the sevoflurane group. Serum lactate levels exhibited a significant time-by-group interaction with a greater reduction in the remimazolam group (P = 0.021). Remimazolam provided arterial oxygenation and hemodynamic stability comparable to sevoflurane during OLV. The greater reduction in serum lactate levels with remimazolam suggests its potential metabolic or immunomodulatory advantages that warrant further investigation.
Comparison of remimazolam and sevoflurane on arterial oxygenation during one-lung ventilation in thoracoscopic surgery: a randomized controlled trial · PMID 41101786Although alveolar recruitment strategy (ARS) before one-lung ventilation (OLV) is beneficial in intraoperative oxygenation, the optimal protocol remains unestablished. As lung ultrasound (LUS) has been used recently, we designed a randomized controlled trial to compare preemptive LUS-guided ARS with conventional ARS in thoracic surgery. Patients aged 20-80 years scheduled to undergo lung resection surgery with OLV were randomized into 2 groups: (i) control group receiving conventional ARS and (ii) LUS group receiving LUS-guided ARS. ARS and modified LUS scoring were performed 5 min after intubation and before emergence. Arterial blood samples and respiratory parameters were collected every 30 min during OLV. The primary outcome was the incidence of intraoperative hypoxaemia (SpO2 < 95%). In total, 166 patients were included. The incidence of intraoperative hypoxaemia was 1.2% in the LUS group and 14.3% in the control group [risk ratio (95% CI) 0.09 (0.01-0.64), P = 0.002]. However, the incidence of intraoperative severe hypoxaemia (SpO2 < 90%) was not significantly different [1.2% vs 6.0%, risk ratio (95% CI) 0.20 (00.02-1.72), P = 0.213]. In the LUS before emergence, higher atelectasis score (P = 0.005) and more significant atelectasis (P = 0.031) was observed in the control group. Postoperative adverse outcomes were comparable between both groups. LUS-guided ARS before OLV was more effective than conventional ARS in preventing intraoperative hypoxaemia during thoracic surgery. Effectiveness of ultrasound-guided alveolar recruitment in thoracic surgery with one-lung ventilation: a randomized-controlled trial · PMID 40581064Despite the physiological advantages of positive end-expiratory pressure (PEEP), its optimal utilization during one-lung ventilation (OLV) remains uncertain. We aimed to investigate whether individualized PEEP titration by lung compliance is associated with a reduced risk of postoperative pulmonary complications during OLV. We searched PubMed, Embase, and the Cochrane Central Register of Controlled Trials until April 1, 2024, to identify published randomized controlled trials that compared individualized PEEP titration by lung compliance with fixed PEEP during OLV. The primary outcome was a composite of postoperative pulmonary complications. Secondary outcomes included clinical outcomes (pneumonia, atelectasis, ARDS, cardiovascular complications, mortality), respiratory mechanics, gas exchanges, and hemodynamic variables. Subgroup analyses were conducted for the primary outcome according to the PEEP titration method (dynamic compliance vs. driving pressure/static compliance, stepwise decremental vs. incremental strategy). Ten trials involving 3426 patients were included. Compared with fixed PEEP, individualized PEEP titration by lung compliance was associated with reduced risk of a composite of postoperative pulmonary complications (eight trials, 3351 patients, risk ratio [RR] 0.55, 95% CI 0.38-0.78). Subgroup analyses suggested the association was evident in the subgroup with titration by dynamic compliance rather than driving pressure/static compliance and in the subgroup with PEEP titration by stepwise decremental but not stepwise incremental strategy. Individualized PEEP titration by lung compliance was also associated with a reduced risk of pneumonia (RR 0.71, 95% CI 0.52-0.96) and atelectasis (RR 0.63, 95% CI 0.45-0.88), higher dynamic compliance, PaO2, PaO2/FiO2, and lower driving pressure. The individualized and fixed PEEP groups did not differ in ARDS, cardiovascular complications, mortality, peak pressure, plateau pressure, PaCO2, heart rate, and mean arterial pressure. Compared with fixed PEEP, individualized PEEP titration by lung compliance is associated with a reduced risk of postoperative pulmonary complications during OLV, especially in PEEP titration by dynamic compliance or stepwise decremental strategy. It is uncertain whether individualisation of the perioperative open-lung approach (OLA) to ventilation reduces postoperative pulmonary complications in patients undergoing lung resection. We compared a perioperative individualised OLA (iOLA) ventilation strategy with standard lung-protective ventilation in patients undergoing thoracic surgery with one-lung ventilation. This multicentre, randomised controlled trial enrolled patients scheduled for open or video-assisted thoracic surgery using one-lung ventilation in 25 participating hospitals in Spain, Italy, Turkey, Egypt, and Ecuador. Eligible adult patients (age ≥18 years) were randomly assigned to receive iOLA or standard lung-protective ventilation. Eligible patients (stratified by centre) were randomly assigned online by local principal investigators, with an allocation ratio of 1:1. Treatment with iOLA included an alveolar recruitment manoeuvre to 40 cm H2O of end-inspiratory pressure followed by individualised positive end-expiratory pressure (PEEP) titrated to best respiratory system compliance, and individualised postoperative respiratory support with high-flow oxygen therapy. Participants allocated to standard lung-protective ventilation received combined intraoperative 4 cm H2O of PEEP and postoperative conventional oxygen therapy. The primary outcome was a composite of severe postoperative pulmonary complications within the first 7 postoperative days, including atelectasis requiring bronchoscopy, severe respiratory failure, contralateral pneumothorax, early extubation failure (rescue with continuous positive airway pressure, non-invasive ventilation, invasive mechanical ventilation, or reintubation), acute respiratory distress syndrome, pulmonary infection, bronchopleural fistula, and pleural empyema. Due to trial setting, data obtained in the operating and postoperative rooms for routine monitoring were not blinded. At 24 h, data were acquired by an investigator blinded to group allocation. All analyses were performed on an intention-to-treat basis. This trial is registered with ClinicalTrials.gov, NCT03182062, and is complete. Between Sept 11, 2018, and June 14, 2022, we enrolled 1380 patients, of whom 1308 eligible patients (670 [434 male, 233 female, and three with missing data] assigned to iOLA and 638 [395 male, 237 female, and six with missing data] to standard lung-protective ventilation) were included in the final analysis. The proportion of patients with the composite outcome of severe postoperative pulmonary complications within the first 7 postoperative days was lower in the iOLA group compared with the standard lung-protective ventilation group (40 [6%] vs 97 [15%], relative risk 0·39 [95% CI 0·28 to 0·56]), with an absolute risk difference of -9·23 (95% CI -12·55 to -5·92). Recruitment manoeuvre-related adverse events were reported in five patients. Among patients subjected to lung resection under one-lung ventilation, iOLA was associated with a reduced risk of severe postoperative pulmonary complications when compared with conventional lung-protective ventilation.
